Please send your student to the Athletic Secretary's Office in the M Building to verify whether or not you have a current physical on file. All physicals are valid for one year from the date your physician signs on the back of the physical form.

Please be informed that students must meet academic requirements. A 2.0 GPA is required and a student will be placed on academic probation if they have 2 "F"s, 1 "F" and 2 "D"s, or 4 "D"s. Probation requires no practices or games. If a student must be on campus after school hours, they must report to study hall.

NO student is to be on a practice or playing field without having FHSAA's required EL-2 form (physical) and the FHSAA's EL-3 (consent and release) filled out.

There are four papers that need to be signed and returned to Mrs. Mary Lindstam in order for your physical information to be complete.

These include:
     - St. Vincents - HIPPA consent and release
     - Athlete Information page
     - EL-2 physical both pages front and back
     - EL-3 Cosnet and release from FHSAA

If your student is a foreign exchange or a home-schooled student, additional information is necessary to be able to compete in any sport.
